These are The Surrogacy Group’s surrogate screening reuirements. We do follow ASRM (American Society of Reproductive Medicine) guidelines, which is listed as follows for our surrogates: 


  • Be between the ages of 20-40

  • Have given birth to at least one child of her own

  • Have had uncomplicated pregnancies and deliveries

  • Be height-weight proportionate 

  • Be in a stable living situation

  • Have the support of her spouse or partner (when applicable)

  • Be a citizen or permanent resident of the United States

  • Be willing to take medications via injection

  • Not smoke or take illegal drugs, or be exposed to second hand smoke

  • Be willing to refrain from alcohol throughout the pregnancy

  • Not have any psychiatric illness

  • Have good communication skills

  • Be reliable, responsive, and mature

  • Have reliable transportation to travel to doctor’s appointments and screening

  • Be able to provide contact information for past OBs or family doctors

  • Enjoy pregnancy and be motivated by the wish to help others create or add to their family





Every clinic has slightly different testing requirements. But, below is a list of the testing that many clinics require. 


  • Current pap smear and annual physical

  • Infectious Disease Testing- to ensure that all parties are clear of transmittable disease such as AIDS, herpes, hepatitis, etc.  If you are married or have a partner, your husband/partner will also have to have infectious disease testing. 

  • Hysteroscopy-visualization of the uterine cavity thru a thin scope inserted through the cervix or dye introduced into the uterus and fallopian tubes to determine the shape and size of the uterus and whether or not the fallopian tubes are clear. 

  • Psychological Testing and evaluation- You will be evaluated by a therapist familiar with surrogacy issues. The evaluation will include a minimum of an one hour one-on-one session with the therapist exploring the surrogate’s motivations, attitudes and commitment to the surrogacy process. Many fertility clinics allow this evaluation to be done over the phone.
